How to refer a patient:

Referrals for endodontic treatment must be completed electronically using the following form.

Please follow the instructions on the form and email this to [email protected] who will triage and appropriately send on to us after they have assessed it for suitability.

Please note we CANNOT accept any Level 2 Endo referrals directly to our practice.
