How to refer a patient:

Referrals for endodontic treatment must be completed electronically using the following form.

CLICK HERE for the Endodontic Referral form and guidance. This should be sent to [email protected] who will if appropriate send on to us after they have assessed it for suitability.

Please note that at this time all urgent endodontic cases can be sent directly to us at the practice using [email protected]  Please mark the email as urgent (see guidance for further details).

All non Urgent cases must be sent directly to the hospital ([email protected] ) for triaging if appropriate send on to us after they have assessed it for suitability as usual.
